Abstract
This dissertation is made up of three parts. The first two parts comprises of Ten Gynaecological and Ten Obstetric conditions and commentaries. These are a few selected cases of the conditions managed by the author during his period of residence in the department of Obstetrics and Gynaecology. In each case a thorough history, physical examination and relevant investigations are presented. At the end of each case a commentary is written on one or two silent features of the presented case. This is in form of review of literature, controversies, and the author's opinion. The third part is comprised of a long obstetric commentary. In this a prospective descriptive study on the magnitude, obstetric characteristics and pregnancy outcome of preterm premature rupture of membranes at Muhimbili Medical Centre was conducted. The study revealed a prevalence of preterm premature rupture of membranes to be 3.6%. Majority of them (59.1°l0) were married, either housewives or peasants. Low birth weighs rate (below 25008) was particularly high due to prematurity. RDS, neonatal infective morbidities, and perinatal or early neonatal deaths were particularly common. Mild maternal infective morbidities were commonly found but the severe forms were particularly rare. There was no maternal death among the studied women. Recommendations are made that further studies need to be done to establish association factors, factors contributing to infective morbidities and establishment of the gestational age where active management can be advocated.
