Abstract
This dissertation consists of ten surgical conditions which have been divided into three parts. Part one consists of five conditions in General Surgery, part two has three conditions in Orthopaedics and Trauma and part three has two conditions in Paediatric Surgery. In each condition an appraisal by a summary of the characteristic features of all the patients in the study population and finally the relevant literature on the subject has been reviewed. In Part One the following cases have been discussed: (a) 10 patients with duodenal ulcer perforations were seen and managed in surgical firm II in Muhimbili Medical Centre from July 1980 to July 1982. There were 9 males and 1 female and the age ranged between 25 years and 60 years. The majority were in the 20-39 years age group. The presenting symptoms and signs were analysed. (b) 62 patients with benign prosthetic hyperplasia were operated in General Surgical Firm II of Muhimbili Medical Centre between July 1980 and June 1982. The peak incidence was found in the 6th and 7th decades with a mean age of 65.3 years. 70.9% of the patients presented with urinary retention either in the acute or chronic forms. 25.8% had evidence of renal function impairment. (c) 21 patients with thyroid swelling were managed by Firm II between July 1980 and June 1982. There were 18 females and 3 males. The age ranged from 1 years to 60 years with a peak incidence in the 2nd and 3rd decades. Two patients had clinically toxic goitre and the remaining 18 had a clinical index compatible with non-toxic goitre. (d) 185 inguinal hernia operations were performed in general surgery firm II between July 1980 and June 1982.Of these 30 that were performed by the author had records available for analysis. All the patients were males with ages ranging from 13 years to over 60 years. Two peak incidences in the 12-30 year age group and 50-60 year age group. There was a right inguinal hernia predominance. (e) 15 patients who presented with ruptured spleen wee managed during a period of 30 months from July 1980. All the patients were males with ages ranging between 12 and 74 years the majority being in the 21 to 40 year age group. All the splenic ruptures were traumatic. Abdominal pain was the commonest symptom. In Part two, the conditions discussed are the following: (a) 34 patients were managed for fractured femora in a period of 3 months at the orthopaedic and Trauma surgery Department, Muhimbili Medical Centre. 24 of these were males and 10 females. The age range was 10 to 94 years with a mean of 35.7 years femoral fractures were more common in the second and third decades. (b) 56 patients with head injuries were treated in the department of Orthopaedics and Trauma, Muhimbili Medical Centre. Their age ranged between 20 and 39 years. 71.4% of the patients sustained head injuries in motor traffic accidents. (c) 8 patients with tuberculosis of the spine were managed in the Orthopaedic and Trauma Department of Muhimbili \medical Centre for a period of 3 months. Most of the patients were in early adulthood. 4 were males and the other 4 females. They presented chiefly with constitutional symptoms of fever, loss of weight andanorexia. In Part three the surgical conditions discussed are following: (a) 41 children were managed for burns in the Paediatric Surgery Unit of the Muhimbili Medical Centre. There were 21 males and 20 females with ages ranging from 6 months to 9 years. Most of these patients came from areas generally regarded as of low socio-economic status in Dar Es Salaam. Hot liquids were the commonest cause of burn. (b) 10 children with osteomyelitis were managed at the Paediatric Surgery Unit of Muhimbili Medical Centre during the months of September to November 1982. There were 8 males and 2 females with ages ranging from one month to 9 years. Six of them were 3 years old or less. Fever, local swelling and discharging sinuses were the common presentations.
