Abstract

The utilisation of Computerised Hospital Information System (CHIS) by clinicians at Muhimbili National Hospital (MNH) has been estimated to be at 50% making the clinician modules among the most underutilized modules of all twenty four modules constituting the system. No scientific studies have been carried out to explore barriers to adoption of these modules as well as determine the extent of utilisation of the system by clinicians. This became the rationale for the current study. An explorative-descriptive cross-sectional study involving seven hundred clinicians randomly selected by using a circular systematic sampling technique was conducted to establish barriers. Data was analysed using Statistical Package for Social Sciences (SPSS). A multistage logistic regression analysis was used to establish determinants and barriers to system utilisation. The findings from this study show that the utilisation of system among clinicians was higher than the estimate by the hospital with over seven in ten clinicians (73.8%) using the system frequently. Ever use of CHIS among clinicians was very high at 82.1% with current users making 97.7% of ever users of the computerised hospital information system. Analysis of system quality, information quality, service quality, and usage and user satisfaction clearly indicates a set of barriers to system utilisation which are discussed in detail in thisdissertation. This study has revealed a higher rate of system utilisation among clinicians than previously estimated. Barriers to system utilisation include technical, critical, supportive and behavioural barriers. There is a need for management to address barriers raised in this study for the system to gain a positive image among clinicians and increase its utilisation.
