Abstract
This dissertation is made up of three parts. Part one consists of management of 10 gynaecological conditions, as indicated in the contents. Part two consists of management of 10 obstetric conditions as shown in the contents. The management of all these twenty conditions was according to the standard management protocol. Because of limited facilities same investigations could not be done in the management of some of these patients. After each presented patient there is a commentary about it and recommendations as to the management of such patients in places under-privileged of facilities and in those places with adequate facilities. Part 3 is a research paper on a common condition of premature rupture of fetal membranes. This part shows the prevalence and outcome of pregnancies complicated by premature rupture of membranes. This is a prospective study carried out at Muhimbili Medical Centre for 6 months ending August 31st 1887. There were 6660 deliveries. 302 had diagnosis of PROM giving a prevalence of 4.5%. 115 patients were included in the study but 12 were excluded for analysis. Expectant management improved maturity and 27% achieved maturity without any serious maternal complications associated with such management. Induction by oxytocin for term pregnancies reduced the PROM delivery interval and thereby improved the patient turnover. This reduced hospital spending and reduced ward congestion. 95% delivered within 24 hours of PROM in the induced group. 75% of the non caesarean section rate was particularly high in the induced group. There were 34 preteen deliveries. 3 were stillbirths and 3 were 1st week neonatal deaths. The rest were term deliveries of these 3 were stillbirths giving the perinatal mortality rate of 77.7 per 1000 deliveries. Slightly lower than the overall perinatal mortality rate in the same period at the same institution. References are given after such case presentation and after the research paper.
