Abstract
A total of 58 cases and 66 controls were seen in a case-control study aimed at studying the role of Neisseria gonorrhea and Chlamydia trachomatis in the causation of sexually transmitted acute pelvic inflammatory disease (ST-APID). Endocarvical purulent discharge lower abdominal pain and positive cervical excitation test were the most frequent sysmptoms and signs, being reported in all cases. Trichomonas vaginalis was seen in 50% of cases and 13.6% of controls, the difference being statistically significant (P0˂. 005). N. gonorrhoeae was isolated in 37.8% of cases and 12.1% of controls, the difference being significant (P˃0.2). Serology showed that C. trachomatis is common in Dar es Salaam whereas direct microscopy indicates that it is rare. This discrepancy was difficult to explain, but one possible reason in that the study included many women who had experienced many chlamydial infections before and become seropositive. This study failed to associate C. trachomatis with acute PID, while N. gonorrhoeae has been shown to be a common of ST-APID.
