Item Chronic posttraumatic Osteomyelitis: current Surgical Management concepts(University of Dar es Salaam, 1997) Mcharo, Cuthbert NathanTitle; Chronic post-traumatic Osteomyelitis, Subjects: 54 Patients with chronic osteomyelitis of long bones surgically treated at Kantonsspital Liestal, Switzerland between January 1992 and December 1995. Objective: The objective of the study was to determine the surgical treatment modality effective in the eradication of infection in chronic osteomyelitis so as to break the vicious circle of recurrent infection - readmission - repeated sequestrectomy and saucerizacion. Summary: Chronic osteomyelitis remains a difficult condition to treat despite recent advances i n surgical technic and availability of broad spectrum antibiotics. Several factors have been advanced for chroniciry and recurrence of the infection. 10 in the developing countries, Tanzania included, the condition continues to have high prevalent rate because of poor initial management of acute osteormyelitis and late as well ,as poor technic of open fractures treatment. Inappropriate and inadequate antibiotic therapy compounds the problem. Commonly the: debridement and saucerization procedures are limited and inadequate resulting in recurrencies. A radical approach in treatment of the condition is done in the Kantosspital Liestal Switzerland with excellent end results. Between January 1992 and December 1995 a total of 54 patients with chronic osteomyelitis of long bones were treated in this hospital using a radical approach. There were 47 male and 7 female. Tibia was the commonly affected bone (703) mainly because of poor soft tissue cover and high frequency of open fractures.Soft issue plastic procedures were necessary in 6 patients to cover the large defect caused by radical debridement. Eradication of the infection was attained in 52 patient (96 %) ac a mean follow-up period of 38 months.Item Ten common surgical conditions(University of Dar es Salaam, 1993) Mcharo, Cuthbert NathanThis work consists of three sections, namely General Surgery, Urology and Orthopaedics and Trauma. General Surgery is comprised of five common conditions as seen at Muhimbili Medical Centre, Urology of three such conditions and Orthopedics and Trauma two. A typical case is presented for each condition, and literature is reviewed. A conclusion is drawn after discussion in each condition.
