Item Obstetric and gynaecologic cases and commentaries(University of Dar es Salaam, 1993) Magotti, Robert FThis dissertation has 3 parts. As shown in the contents both Part one and two are each made up of 10 gynaecological and 10 obstetrical cases and commentaries respectively. All these 20 case reports and commentaries were managed by the author under the supervision of senior staff members it the department. For each case a thorough history, examination, investigations and treatment have been narrated. Cases were managed according to standard laid down in management protocols though investigations were at times not exhaustive because of limited facilities. After each case there is a commentary followed by relevant references. A long gynaecological commentary is in Part three. A prospective study on cost effectiveness of managing abortions at Muhimbili Medical Centre was done whereby Manual Vacuum Aspiration (MVA) was compared to Evacuation by Curettage (E by C). MVA is a newly introduced technique of managing abortions at the centre. Its cost effectiveness in comparison to E by C was tested prior to its wider acceptance in the country. A total of 199 patients were interviewed and managed, MVA had 107 respondents and E by C had 92 respondents. The study revealed that the total duration of the procedure, total pre evacuation waiting times and total duration of hospital stay for the study group was 10.96 hours, 15.59 days and 21.23 days less, respectively for MVA patients. Resource utilization by both techniques was analyzed and revealed that MVA was cheaper by 763.07 Tsh. when compared to E by C. It concluded from the study that MVA was more cost effective than E by C in our health care delivery system.
